  • When can I enroll?
      • All benefit eligible employees: during Open Enrollment.
      • New Hires: within 30 days from your date of hire
  • How do I enroll?
      1. CLICK HERE to schedule an appointment with a Benefits Counselor.
      2. A Benefits Counselor will call you at your scheduled appointment time.
  • When will my benefits be effective?
      • All employees after Open Enrollment:  10/01/2024
      • New Hires: 1st of the month following 60 days from date of hire
  • How do I file an Aflac claim?
      • Online claim form:  CLICK HERE to go to the Aflac Group Insurance site where you can download a claim form or file your claim online.
      • Paper claim form:  Download the appropriate claim form below. Then complete and submit your completed claim form to Aflac.  Instructions are on the claim forms.
