How do I file an Aflac claim?

- Online claim form: CLICK HERE to go to the Aflac Group Insurance site where you can download a claim form or file your claim online.
- Paper claim form: Download the appropriate claim form below. Then complete and submit your completed claim form to Aflac. Instructions are on the claim forms.
